The incident date is not recorded in official sources. What is known comes from MSG Clyde Bonnelycke — D Company's Range Platoon sergeant — who was present, and who decades later spent a day with SP4 Sarah's son sharing what he remembered. He brought a copy of a hand-drawn map marking the spot.

SP4 Hugh Henry Sarah was out front during a ground combat mission near the river by LZ Ike when an explosive detonated. He suffered numerous devastating injuries. He lingered approximately twelve days.

SP4 Hugh Henry Sarah died of his wounds on September 23, 1969. He was 24 years old. His wife Vickie was expecting their first child around the same week.

His name is on the Wall at Panel W17, Line 1.
